The Sports Medicine staff at SUNY Buffalo State University is
comprised of four Athletic Trainers (ATs) who work in conjunction with our Team Physicians of UBMD Orthopaedics and Sports Medicine, Buffalo Spine and Sports Medicine, and SUNY Buffalo State University's Weigel Health Center.

ATs are highly qualified, multi-skilled health care professionals who render service or treatment, under the direction of or in collaboration with a physician, in accordance with their education, training and the state's statutes, rules and regulations.  As a part of the health care team, services provided by ATs include primary care, injury and illness prevention, wellness promotion and education, emergent care, examination and clinical diagnosis, therapeutic intervention and rehabilitation of injuries and medical conditions. Athletic training services are available to all current intercollegiate student-athletes at SUNY Buffalo State University.

On behalf of the SUNY Buffalo State University Sports Medicine Staff, we would like to welcome you to campus as a prospective member of our athletics program. In order to comply with NCAA Division III health and safety regulations, there are multiple forms all incoming and returning student athletes must complete prior to participation in any athletic-related activity at SUNY Buffalo State University. All pre-participation forms on the Campus Portal, physical, and sickle cell status need to be reviewed before one's sport participation.
 
Requirements for Athletic Participation: 

1. Completed pre-participation forms in the Campus Portal  
  • Complete all “required” fields and forms 
  • Must be completed by June 23, 2025 (returning and incoming registered student-athletes); August 1st, 2025 for all student-athletes (regardless of status) 
2. Physical exam completed on the SUNY Buffalo State University Athletics Physical Form (PDF) or approved forms from your medical provider submitted
  • Previously accepted physicals expire 1 year from the date of the exam 
  • Please upload your completed physical to your Campus Portal or have your primary care provider fax it to SUNY Buffalo State University Sports Medicine at (716) 878-3536 
3. Proof of your sickle cell status uploaded to your Campus Portal  

NCAA Mandatory Sickle Cell Trait Testing
  • Please work with your local primary care provider to get the script necessary for this testing if you do not know your status.   OR
  • The NCAA developed a relationship with Quest Labs in order to get sickle cell testing completed for the coming year. Please use this link: Quest Labs.  OR

4. Proof of Insurance Uploaded to your Campus Portal
5. Attendance of a Pre-Participation meeting with the Sports Medicine Staff 
  • Date/Time TBA 
7. Please bring an extra dose of any emergency medications (Rescue Inhaler, EpiPen, etc.) that you are prescribed. 
  
Failure to comply with all the above stated Sports Medicine requirements will result in a delay of your medical clearance to participate in athletics.
