Prior to competing for a Buffalo State Athletics team, all student-athletes must complete all medical and compliance paperwork and be cleared for participation by the athletics training staff as well as the director of compliance. In addition, all student-athletes must sign the Student-Athlete Agreement:
> CODE of CONDUCT:
To participate in athletics I will accept responsibility for my personal conduct and accept the College’s standards and policies. I will pride myself on promoting exemplary sporting behavior.
> ACADEMIC ELIGIBILITY:
To participate in athletics, I must carry 12 credits per semester and be in good academic standing as defined by Buffalo State College. A student-athlete not in good academic standing, one who has been at Buffalo State for at least two semesters and has a cumulative GPA of less than 2.0, will be ineligible for intercollegiate athletics. I have an obligation to maintain communication with my professors and abide by policies regarding missed work and class attendance due to competition. Â
> ELECTRONIC FORMS of COMMUNICATION:
I am aware that electronic communication (ie Facebook, Twitter, Instagram, etc.) is considered a public forum and information posted may be viewed by anyone. Information posted electronically may be monitored and may be brought to the attention of conduct officials when they are seen as possible violations of the Student Code of Conduct.
> TRIP CONDUCT:
As a student-athlete, I understand that I am required to travel to and from athletic competitions with the team. Permission from my Head Coach must be obtained to travel with my parent(s)/guardian(s). A violation is subject to immediate disciplinary action. I am aware that I represent SUNY Buffalo State while on an official trip and I will conduct myself in a positive manner.
> HAZING:
I fully understand the College’s zero-tolerance policy towards hazing and agree not to engage in such activities. The consequences for such activities may result in suspension, probation or expulsion.
> GAMBLING:
I understand engaging in such activity is a violation of NCAA policy and suspension will result.
> TOBACCO:
My use of tobacco may result in suspension and/or dismissal from the team.
> ALCOHOL:
I understand the general laws of New York, the College and Athletics Department regarding alcohol use. Violations of civil laws and policy may result in suspension from competition and may be cause for dismissal from the team.
> DRUG POLICY:
My use, possession or distribution of narcotic or dangerous drugs as defined by state and federal laws is prohibited. I understand that violations may result in suspension from competition while completing a mandatory education program with our counseling center. A second offense may result in dismissal from competition for one full calendar year. Distribution will result in immediate expulsion from the College.
> EQUIPMENT ROOM POLICY:
I am responsible for all equipment/apparel issued to me by the Athletics Department. All items are the property of the College and will be respected and returned at the end of the season. It is my obligation to work with equipment room personnel or my coaches to handle all needed repairs, adjustments, replacements and exchanges. I will address such needs in a timely and responsible fashion. It is also my obligation to return apparel immediately following contests for cleaning.
> MEDICAL CARE:
I understand that with athletic participation comes inherent risk of injury. It is my obligation to communicate and work with the Sports Medicine Staff of the College with regard to any health/injury issues, evaluations, treatments, care, rehabilitation and recovery.
